COMMUNITY BASED SERVICES

 

Campagna Academy's Community Transition Program (CTP) is designed for children with behavioral or emotional problems that have begun to transition out of the department of corrections placement and back into their home communities. This transition program assists with increasing stability or success in the home, school, or community. We focus on individualized academic needs, emotional adjustment, and family support.  CTP services are consistent with the young person's ability, as well as his or her social and emotional functioning levels. 

 

CTP meets the needs of the following circumstances:

n Youth transitioning back to a home after DOC commitment

n Youth at risk of reoffending

n Individual strength-based treatment plan

n Linkage to community resources

n Individual treatment focusing on adjustment back home and into the
     community, social impairment, and positive decision making

n GED program linkage

n Employment preparation and opportunities

n Development of social skills

n Enhancement of self-esteem

n Teach personal responsibility to promote successful community living

 

 

Program Specifics:

n 10 to 18 year-old males and females

n Individual and family support and case management

n Individualized support plan

n Development of pro-social skills

n Vocational, Community, and Educational linkage
